Telomerase, a eukaryotic ribonucleoprotein (RNP) complex (26– 33), helps to stabilize telomere length in human stem cells, reproductive cells and cancer cells (35, 36) by adding TTAGGG repeats onto the telomeres using its intrinsic RNA as a template for reverse transcription .
Inhibiting telomerase caused tumor cell death but also led to alternative lengthening of telomeres (ALT) independent of telomerase. In this model, telomerase activation occurs after the selection of mutations that stimulate abnormal cell proliferation, and, thus, telomerase activation may be a relatively late event in tumor initiation that allows for genome stability after the acquisition of cancer-associated mutations. It is believed that cancer occurs because a genetic mutation can trigger the production of an enzyme, known as telomerase, which prevents telomeres from shortening. While every cell in the body has the genetic coding to produce telomerase, only certain cells actually need it. Telomerase has been a prime target for the development of effective therapeutics against cancer as it is expressed in the majority of cancer types as well as in cancer stem or stem-like cells.

More work on the tumor specific regulation of telomerase activity might provide either more opportunities for telomerase inhibition, or more skepticism, as a tumor cell might possess mechanisms for upregulating telomerase activity in the presence of inhibitors.

The powerful and complex impact of telomere dynamics in model organisms and humans reflects the crucial role of telomere function in processes of genomic instability, organ homeostasis, chronic diseases, aging, and tumorigenesis. Telomerase therapy is not only unlikely to result in an increased risk of cancer but is likely to lower the risk of cancer compared to age-matched patients not treated with telomerase therapy. Review of available data suggests that cancer should not be considered a significant risk to patients undergoing telomerase therapy, but the actual outcome will require human trials.
